Application Procedures 1
Obtain Application Form
Application forms together with notes on application can be obtained free of charge at all public housing estate offices, District Tenancy Management OfficesSham Shui Po Housing Information Centre, Applications Sub-section of Housing Department and District Offices of the Home Affairs Department. For request of application forms by post, the applicant has to submit a written request to the Public Rental Housing Applications Sub-section and enclose a self-addressed envelope with the correct postage affixed.

Submit Application Form
Each applicant can submit one application only. Individual family members should not be included in more than one application. Duplicate applications will be cancelled. The applicant should submit the completed application form to the Public Rental Housing Applications Sub-section together with the following documents for preliminary verification of their eligibility for registration:
1. Identity and Relationship Proof for Applicant and Family Members
(i) Photocopies of identity cards, marriage certificates, birth certificates or documentary proof of family members' relationship. If the marriage and / or birth took place in China where certification had not been applied for or issued by the relevant authorities, a Notary Public Certificate may be considered a substitute. If the marriage took place in Hong Kong through old Chinese marriage custom, please declare. If there are discrepancies in the spellings of names, please submit documentary proof;
 
(ii) Copies of documents proving Hong Kong residence for children who were not born in Hong Kong and have not applied for any identity cards. Copies of adoption documents issued by the Hong Kong Government should be produced for adopted children;
 
(iii) Copies of the divorce certificate and custody order for divorcee and single parent. In case of separation without legal marriage, original copies of the statutory declaration stating the date of co-habitation and separation, and the custody arrangement of the children should be produced. If the spouse has disappeared, a statutory declaration and a copy of the Police Report should be submitted;
 
(iv) Copy of the death certificate should be submitted if the spouse is deceased. If the spouse is not yet residing in Hong Kong, the applicant must state in writing and produce a copy of marriage certificate. If the spouse lives abroad or in the mainland permanently, documents showing the spouse's present place of residence should be submitted;
 
(v) Copy of medical proof issued by a registered doctor to certify the expected date of delivery if any family member included in the application has been pregnant for 16 weeks;
 
(vi) Copy of the entry permit or passport for those who have the right of abode in Hong Kong for less than seven years.
   
2. Income Proof for Applicant and Family Members
(i) The latest (not more than three months ago) employment certificates of the applicant and all working family members stating the basic salary, double pay, bonus and other allowances (the employment and salary certificate should be issued and certified by the employers and bear the chop, name, address and telephone number of the company and the date of issue). The applicant can deduct from his / her income the 5% statutory contribution to Mandatory Provident Fund. Other types of contribution are regarded as part of the income;
 
(ii) For casual workers, they must submit a written declaration on their present jobs and average monthly income bearing their name and identity card number, date of making the declaration and their signature;
 
(iii) Self-employed persons should furnish a photocopy of the Business Registration Certificate with a written statement on the asset and the average monthly profit and income;
 
(iv) Photocopies of the latest medical waivers and social security documents showing the page with name of the recipient and the amount being received if he / she is receiving Comprehensive Social Security Allowance;
 
(v) For persons out of employment, a written statement on the source of financial support (including donation from children and relatives) and average monthly expenditure should be provided. All declarations should be dated and signed by the declarants.
    
3. Net Asset Value Proof for Applicant and Family Members
Copies of documents supporting the net value of different kinds of assets owned by the applicant and family members should be submitted.
 
4. Proof of Address
Copies of any document (such as electricity bill, water bill, etc.) bearing the applicant's name / correspondence address in English for cross checking if the address he / she has entered in English is correct.

Application Procedures 3
Receive Acknowledgement of Registration
All applicants are subject to eligibility vetting before their application is registered. The application form will be returned and no registration will be made if the applicant does not fulfil the eligibility criteria or fails to provide relevant documents together with his / her application form.

All applications are to be investigated by the Applications Sub-section (AS) of the Housing Department. After registration, the AS will send the applicant a blue acknowledgment card bearing the application number allocated to him / her. If the applicant does not receive the card one month after submitting the application form and all the requisite documents, he / she may send in a written enquiry stating his / her name, Hong Kong Identity Card number and correspondence address, or may enquire in person to the Enquiry Counter of the Applications Sub-section.
